Our Care Navigation Team serves as a liaison between KSOSN, partner facilities, dialysis organizations, transplant centers, and surgeons.
They are dedicated to providing outreach and coordination of care to our complex patients. They are responsible for tracking and monitoring a patient through their healthcare journey.

Alex Farfan
Director of Operations
Alex Farfan joined Kidney Specialists of Southern Nevada’s Executive Team in July of 2021. Alex has extensive background in Healthcare Administration.
Alex retired from the United States Air Force after 20 years of service. After retirement, Alex spent 2 years as a Facility Administrator for DaVita. Alex earned a Master of Healthcare Administration degree from Oklahoma State University.
In addition, Alex loves to golf, cook, and travel.
Tommy Ahsan
Director of Value Based Care
Tommy Ahsan joined Kidney Specialists of Southern Nevada’s Executive Team in November of 2021. He has an extensive background in data analysis and organizational behavior.
Tommy was previously the Director of Analytics at Silver State ACO.
He graduated with a master’s in management from Duke University, while also completing his Bachelor of Business Administration at the University of Miami. Tommy is currently pursuing an MBA at Johns Hopkins University.
Tommy is an avid mixed martial arts fan and practices jiu jitsu. He lives with his wife and 3 dogs in Summerlin.
