What to expect

Visiting any physician can be intimidating and uncomfortable. Let us help by sharing these tips to help you navigate the new patient and follow up appointment processes.

When making an appointment, write down a list of things you need to tell the provider as well as the names and dosages of any medication you may be taking. It is important that we know all the medications you are taking. Remember that the information you provide allows your doctor to better establish a diagnosis or determine what treatment is most appropriate. Be honest and do not withhold information. Utilize our office personnel, nurses, and medical assistants as they can be additional resources for information. Do not hesitate to ask them questions or express any concerns you may have.

The following simple tips will help you make the most of your visit:

Appointment Setting Tips

After you have scheduled your first appointment you will receive the New Patient Registration paperwork via email or FormDr. This registration paperwork is required for your first appointment. When scheduling your appointment please make sure to pick a day and time that you will be able to make.

Before your First Appointment

  • Have all medical records from previous doctors sent to our office.
  • Prepare a list of questions for your provider.
  • Prepare a list of all medications and dosages including over the counter as well as any drug allergies.

Day of your First Appointment

  • Bring your id and insurance card.
  • Bring your fully completed new patient registration paperwork.
  • Arrive at least 20 minutes early to finish completing any additional forms.
  • Call ahead if you are running late. We do allow a 15-minute grace period, but please be aware that if you arrive late, your wait time may be longer. We will do all we can to accommodate your appointment and to minimize the need to reschedule.
  • If you need to cancel or reschedule, we do ask that you give a 48-hour notice, so we are able to offer the appointment time to another patient.

During your First Appointment

  • Provide list of current medications.
  • Answer any questions the provider has as accurately and honestly as possible.
  • Review your list of questions with the provider.
  • Ask any questions that may come up during your visit.
  • Be sure to write down or ask for written instructions to help understand the provider’s care instructions.

After your First Appointment

  • Make sure you receive your prescription and understand the medication instructions.
  • Checkout at the front desk and schedule your next appointment if needed.

Before your Follow up Appointment

  • Get your lab orders drawn and any other testing that your provider referred you for.
  • Prepare a list of questions for your provider.
  • Prepare a list of all medications and dosages including over the counter as well as any drug allergies.

Day of your Follow up Appointment
